Intermediate Procedures

Cholecystectomy plus exploration of the bile duct.

This consists of the previously mentioned procedure plus the removal of stones in more advanced cases where the stones have already passed into the ducts that descend from the liver and become trapped in the main bile ducts (choledocholithiasis). This is a condition that, if not treated in time, can cause serious infection of the ducts and liver (cholangitis) and even irreversible liver failure with the death of the patient. This procedure can be performed with three to four small incisions.

Diverticulectomy.

Sometimes there may be abdominal pain on the left side in young patients or adolescents. This pain is very similar to that which occurs in cases of appendicitis and may also be accompanied by nausea, vomiting, fever and/or bleeding when evacuating. The condition may correspond to Meckel’s diverticulitis. This can be effectively resolved by means of laparoscopy by introducing sharp staplers. The evolution of the patients is sensational. They require little hospitalization and only three small holes in the abdomen are used for their treatment. It should be noted that the diagnosis is not very easy to make, however a diagnostic laparoscopy can end the uncertainty and solve the problem without the need for exploratory laparotomy (opening the abdomen completely).

Laparoscopic Hematobilioma Drainage.

When there is abdominal pain secondary to previous surgical procedures (including open ones by traditional surgery) the drainage (extraction) of blood, serous, biliary or purulent collections from the cavity can be performed laparoscopically regardless of the time of evolution of these or their location. In this case the patient presents seven days after surgery mediated by open cholecystectomy with a significant infectious state. Upon examination, a large trapped collection of blood and bile (hematobilioma) was found, which caused the deteriorated and septic (infectious) state of the patient. This method is suitable for patients with inadequate convalescence, since it significantly reduces the pain of a second recovery and practically nullifies the risk of wound infection, conditioning the prompt reincorporation of the patients to their daily life.

Laparoscopic fundoplication for hiatal hernia and gastroesophageal reflux.

It is performed in cases where medical treatment for hiatal hernia and gastroesophageal reflux has not worked at all or not at all and the signs and symptoms of gastroesophagitis (inflammation of the stomach or esophagus) persist, as well as to prevent esophageal damage caused by chronic reflux from continuing and causing cancer. It is the most effective method to resolve these conditions and is currently considered the Gold Standard for hiatal hernia and gastroesophageal reflux. Many gastroenterologists maintain expensive treatments for gastritis and reflux for years. This procedure eliminates the need for these medications in more than 95% of cases and leaves patients symptom-free. It only requires a few months of post-surgical follow-up and the patient forgets about expensive treatments and consultations for life. It only requires four small 0.5mm holes. It is the safest surgery to change the quality of life.

Laparoscopic Tencoff catheter placement.

Patients who suffer from terminal renal failure and are unable to receive hemodialysis due to its costs and high demand and/or patients in whom hemodialysis is no longer possible, are candidates for the placement of a catheter that allows the passage of external solutions that eliminate metabolic products from the body. This procedure is currently performed minimally invasively, which offers many advantages to the renal patient, including less postoperative pain, lower risk of catheter failure since it is placed under direct vision (which does not happen in open surgery), it offers the possibility of placing as many catheters as in open surgery is not possible due to damage to the abdominal wall and secondary adhesions that traditional open surgery causes. It is a very safe method in expert hands and minimizes the possibility of bleeding or injuring the intestine (frequent occurrences in open surgery since it is a blind procedure). It only requires two small 0.5mm holes and the patient can use the catheter immediately. It is placed even in patients who have had multiple open operations since this method finds the ideal place to relocate the catheter.

Varicocelectomy.

Patients who have secondary dilation of the veins that drain the testicles now have the possibility of solving the problem safely and effectively by laparoscopy. It is a procedure that takes a few minutes and is very safe. Mainly the abdominal aesthetics are respected. The patient is discharged on an outpatient basis. It only requires one or two 0.5mm holes.

Anastomosis of the small intestine to the large intestine.

When a patient has an ileostomy due to surgery with large bowel resection, it is possible to “reconnect or reanastomoses” it laparoscopically with all the advantages already mentioned if the patient has the appropriate training and equipment.
